...early as Dec. 17, 1982, the testimony showed, NASA had been concerned enough about the possibility that the O rings might fail during lift-off to designate them as "criticality 1" items, components whose failure would doom the mission. Earlier shuttle flights had indicated that the second ring might be unseated from its groove by the great pressures on the rocket casing during lift-off, and could not always be relied upon as a backup should the first ring fail. Lack of a reliable backup violated a longtime NASA principle. The space agency formally waived the redundancy requirement for these...
...education programs can be implemented immediately, but vague talk of trickle-down cultural regeneration hardly translates into effective action. If the recently awakened concern leads only to a flurry of arbitrary moral judgments, it will fail of its promise...

...song could change to Nobody Knows the Trouble I've Seen if the Administration and Congress fail to deal effectively with a pair of megaproblems: the budget and trade deficits. Government borrowing to cover the budget shortfall could eventually send interest rates shooting back up. Also, the trade deficit could boost interest rates because the U.S. may be forced to borrow more and more money from abroad to finance imports. Heller observed that the Government's record in attacking the twin deficits has so far been "somewhere between lackluster and lousy, but 1986 really looks like a turnaround year...
What might have caused an O ring in the right booster to fail? Panelist Feynman demonstrated one possibility at the public hearing by conducting a simple experiment in front of the TV cameras. He placed a small section of a ring in a C-clamp and submerged it in a cup of ice water. Then, removing the section and releasing it from the clamp, he concluded, "The resilience is very much reduced when the temperature is reduced." That fact may be significant, because the booster joints that the O rings are supposed to seal shift under the enormous stresses...
